Basic Admission information

With regard to your upcoming admission, please note the following:

1. Please contact your medical aid to obtain authorisation with the codes given. Once you have obtained your authorisation, please contact the rooms with the number.

2. Before your surgery, the relevant hospital admission forms that need to be completed will be sent to you. Please either email it to the relevant hospital or take it with you on the day.

3. Brink Anaesthetics information is available at the bottom of this screen. If you did you did not receive the hard copy, please print out all the forms and take the completed forms with you on the day of admission.

4. Please take note that the practice charges 217% which is Discovery Classic rates. Please take note of the amount on the quotation that your medical aid will be paying for your procedure and what your co-payment by the practice will be. Payment arrangements via debit order can be made with our accounts department prior to your procedure date. Contact Alicia at accounts@jjnortho.co.za to make payment arrangements.

5. Kindly note that procedure quotations are an estimated cost. The procedure codes and final amount might change. The main member will be held liable for any amounts not covered by the medical aid.

6. On the day of the procedure, please bring along hard copies of your x-ray images.
