As a consumer-centred organisation we aim to provide the best experience to our patients - before, after and during your interactions with us. If you are booked for a foot or ankle surgery, we would like to understand how your foot/ankle problem impacted your life before surgery, if surgery improved your abilities and how we can better provide service to future customers.

Therefore, we would greatly appreciate your assistance in completing a survey before your surgery.

We will ask you to complete the same questionnaire again at regular intervals after the surgery, but at any time during the study you may choose not to participate without any negative consequences.
